I am voluntarily providing information about my mental health, identity, and preferences so that Therapy Utah can match me with appropriate therapists. I understand this information will be reviewed by Therapy Utah care coordinators and the therapists I am matched with.

I understand how my data is stored and shared.

My intake data is stored securely on Base44 infrastructure. It will be shared only with: (1) the therapists I am matched with, (2) Therapy Utah care coordinators, and (3) Therapy Utah clinical leadership in the event of a safety concern. My data will not be sold, shared with third parties for marketing, or used for any purpose outside of therapist matching and clinical care.

I understand the matching process.

The therapist matches I receive are recommendations based on a structured algorithm — not a clinical diagnosis or guarantee of fit. I understand I can decline any match, request a different therapist at any time, and that the final decision about my care is mine.
